Firstly, you need to choose a modding method, because there are not the same and use different perks:

 

Source modding us confused and difficult one, but it provides many interesting features

ModLoader is much easier to use, but it has less features and is unpopular one

Minecraft Forge is more difficult to develop, but easier to install mods, it is popular and I recommend to start with it.

 

Secondly, you need set up it properly:

Here is a list of application to install:

Java Development Kit 7 

eclipse

Minecraft Forge

Extract the files to folder you want

Windows: Shift + Right Click - Open command window here

Mac: Terminal  - navigate to the directory of the extracted files.

Use this command:

Windows: gradlew setupDecompWorkspace

Mac: Terminal  - /gradlew setupDecompWorkspace

Then run

Windows: gradlew eclipse

Mac: Terminal  - ./gradlew eclipse

Open Eclipse
